How to Deal a Patient with Suspected DVT signs in CSA?
DVT PATHWAY IN PRIMARY CARE
When we clinically suspect possible DVT in CSA, it is very important to blend our questions with the following points in mind.
1. Appropriate History and Examination.
2. Risk factors for DVT
3. Well’s score questions
4. Contraindications for anticoagulation
5. Local referral pathway
6. Support network
EASY STEPS
1. History and Examination
2. Well’s Score undertaken – Low 0 to 1 – DVT unlikely while High 2 or more – DVT likely.
3. If DVT still suspected, Prescribe Rivaroxaban 15 mg BD (21 day supply only)
4. Send Bloods for URGENT D- Dimer and Clotting screen, FBC, UE, LFT, CRP.
5. Check the D-dimer result.
a. If negative – Stop Rivaroxaban.
b. If Positive D- dimer OR High Well’s Score – Refer US scan.
1. If DVT confirmed in scan – Continue Rivaroxaban for minimum 12 weeks.
2. If no significant DVT demonstrated, Repeat US scan in 7 days. If negative – Stop Rivaroxaban.
